You represent and warrant that all information you provide through the FraudReportHub Services—including broker names, NPNs, and descriptions of fraudulent activity—is true and accurate to the best of your knowledge.
Prohibited Use: You may not use FraudReportHub to file false, malicious, or harassing reports. Filing a false report to a government agency may have legal consequences under federal and state law.No Verification: While FraudReportHub provides tools to organize data, we do not independently verify the truth or accuracy of every user-submitted report.3. ROLE AS A FACILITATOR
FraudReportHub acts solely as a facilitator. By using our "Submit" or "Email" features, you are authorizingFraudReportHub to transmit the data you provided to third parties, including but not limited to the CMS, DOJ, HHS-OIG, and state insurance departments. Awgmnt is not responsible for how these third parties process, investigate, or respond to your report.
4. LIMITATION OF LIABILITY
To the maximum extent permitted by law, FraudReportHub and its affiliates shall not be liable for any indirect, incidental, or consequential damages resulting from:
Your use or inability to use the Services.The outcome of any report filed through the platform.Any actions taken by third parties (government agencies or brokers) as a result of your report.Technical errors in the "Auto-Fill" or "Highlight" functionality.5. NO LEGAL ADVICE
